Securamed
is committed to protecting your privacy online. This Privacy Policy
and Agreement ("Privacy Policy") will tell you what information
we collect about you and your use of our website. It will explain
how we protect that information and what choices you have about how
it is used. We urge you to read this Privacy Policy carefully so that
you clearly understand both our commitment to you and your privacy
and how we collect and use any provided information. By using the Securamed
site, you agree to the terms of this Privacy Policy. In particular,
this privacy policy describes:

As a further commitment to protecting your privacy online, we subscribe
to HON (Health on the Net) ethical guidelines, which ensure the confidentiality
of all health information on the Internet. No one, except you and the
individuals you authorize, can access your health information. You
can receive a printed copy of this policy or any part of your personal
record by clicking on the "Print" option under "File"
on your browser.
Under
no circumstances will Securamed release any of your personal information
to any person or company unless ordered by a court of law. We use
state-of-the-art technology to ensure that your information can
be stored and retrieved securely and safely.
Our service is to help consumers store their medical information
in a safe and secure place and our business model is not dependent
on selling any individual's information.

A. Information we collect
There
are a number of ways that you may gain access to a Securamed service,
and how you gain access has some bearing on the information
that is collected from you. You may have directly accessed a tool
from www.Securamed.com, or you may have accessed it from a partner
site. A partner site is a site hosted by a third party that provides
Securamed services.
When
you register for a Securamed service, we DO NOT collect any personally
identifiable information that can be used to identify you or contact
you. We only collect a username and a password assigned by you. In many cases, you are
asked to enter your registration information directly into our forms.
Otherwise, your information may have been entered into our forms had
it already been entered while in the registration process
at a partner site.
The
information provided to Securamed by the partner site is not retained
by Securamed until you complete the Securamed registration by hitting
the "Register" button. If you choose not to complete the
Securamed registration process and exit our site before hitting
the "Register" button, your registration information is
discarded. The partner site will assign you a unique partner-specific
identifier that confirms to Securamed that you are an authorized
member of that partner site. Once a unique partner-specific identifier
is linked to your registration information, this identifier will
always be used whenever you access a Securamed service from that
partner site. If you access Securamed services from multiple partner
sites, you will have a unique partner-specific identifier for each
partner site.
When
you create your medical record, in any language, using Securamed,
you enter your pertinent health information. But since we do not collect your
name, address, email address or any other personally identifiable
information, the health details that you fill out are NOT linked
to you and CANNOT be termed as personal health information.
B. Cookies
Cookies are small computer files that we transfer to your computer's
hard drive that allow us to know how often someone visits our site
and the activities they conduct while on our site. Written as a simple
text file, a cookie helps your browser remember specific information
that the Web server can later retrieve. Some examples of Securamed's
use of cookies include:

Your
browser software can be set to reject all cookies. Most browsers
offer instructions on how to reset the browser to reject cookies
in the "Help" section of the toolbar. If you reject our
cookie, certain of the functions and conveniences of our site may
not work properly but you do not have to accept our cookie in order
to use our website in a productive manner. You should review the privacy policy of
other sites you visit or link to from our site to understand how
these other sites use cookies and how they use the information they
collect through the use of cookies on their own sites. We do not control these third parties'
use of cookies, or how they manage the non-personal information
they gather through them.
C. Children
Securamed's
services and tools are inteded for adult use only. A parent or guardian,
however, may use Securamed's services to establish a medical record
for a minor. The parent or guardian is solely responsible for providing
supervision of the minor's use of Securamed tools. The parent or
guardian assumes full responsibility for ensuring that the authentication
information is kept secure and that the information submitted is
accurate. The parent or guardian also assumes full responsibility
for the interpretation and use of any information or suggestions
provided through Securamed for the minor.
D. Disclosure of your information
Except as set forth in this section or as specifically agreed to
by you, Securamed will not disclose any Personal Information it
gathers from you on our website. We may release Personal Information
to third parties: (1) to comply with valid legal requirements such
as a current law, regulation, search warrant, subpoena or court order; or
(2) in special cases, such as a physical threat to you or others.
In the event that we are legally compelled to disclose your Personal
Information to a third party, we will notify you unless doing so
would violate the law or court order.
Disclosure
to or by co-branded channel Partners
Securamed is a service provider and provides co-branded services
to websites hosted and operated by companies other than Securamed.
You can only access these co-branded services through the Partner
website. The co-branded Securamed pages that you may access through
a Partner website have different registration processes and opportunities
for information collection. Each of these co-branded Securamed sites
has its own privacy policy posted on that site, which explains what
information is disclosed by Securamed to the Partner and vice versa.
If you visit one of these co-branded Securamed sites, please read
the privacy policy that is posted on that site, as well as the individual
privacy policy of the Partner website.
Disclosure
of aggregate information
Securamed may combine, in a non-identifiable format, the health
information you provide with health information from other users
to create "aggregate data" that may be disclosed to third
parties. For example, Securamed may use information gathered to
create a composite profile of all the users of a particular partner
site. Partnering sites may use this information to understand community
needs and to design appropriate programs and activities on their
site. Aggregate data does not contain any information that could
be used to contact or identify you. Securamed will not disclose
your personal health information to any third party without your
prior authorization.
E. Third Parties
All partner sites that feature Securamed services are required to
post a statement that references Securamed and its services. Securamed
further requires partners to receive authorization from users before
Securamed will permit the partner access to any personal health
information that a user provides to Securamed.
Securamed
currently does not accept banner advertisements on its corporate
site (www.Securamed.com). However, if you access a Securamed service/tool
through a partner site, the Web page containing the Securamed service
may feature a banner. This banner is governed by that partner's policies
and practices regarding such advertisements.
Partner banner advertisements do not track your
movement while in a Securamed tool and cannot be used to target
advertising to you based on personal health information you provide
to Securamed. Securamed may have agreements with some partners to
share in the revenue generated from banner advertisements.
Once
you click on a link to a third-party Web site, and leave the Securamed
site, that site's privacy policy, rather than Securamed's, governs
your activities on that site.
F. Internal protection of your records
We employ the following practices to protect your privacy:
Using
passwords to authenticate users before granting access to personal
information
Limiting
access to personal information to authorized users. Securamed lets
you have full control over who has access to your personal health
records. For example, you may permit your relatives or health care
professional to view your records. Only the
person who creates a record can grant such access to other users.
Despite
Securamed's efforts to protect your personal information, in certain
limited circumstances we may be legally compelled to release your
personal information in response to a court order, search warrant,
subpoena or other valid legal action.
Furthermore,
there is always some risk that an unauthorized third party may find
a way to thwart our security systems or that transmissions of your
personal information over the Internet will be intercepted. At
Securamed, however, as all of your records are anonymous and not directly
linked with you, we do not expect any unauthorized disclosure of
your private information.
G. Your privacy choices
Removing
the information
Securamed provides users with the following method for removing their information
from our database, thereby opting not to receive future communications or to
no longer receive our service.

2. Log in to your account using your username and password
3. Go to Administration
4. Click on 'Delete'
In order to correct or update your information, you have the following
options for changing and modifying information previously provided:
Limitations to removing your information
The need to back-up our systems to protect information from inadvertent
loss means that a copy of your personal information may exist in
a non-erasable form that will be difficult or impossible for us
to locate. Nevertheless, we promise that upon receiving your request,
all personal information stored in the databases actively used
for research, daily business activities, and other readily searchable
media, will be deleted.
Changing
your information
1.Visit www.securamed.com and login to your account using your username and password. You can edit/update your information at any time.
2.If for some reason you are unable to correct your information,
e-mail us at support@securamed.com with your request for changes.
Notification
of changes in the privacy policy
If we decide to change our privacy policy, we will post these changes
on this page so that you are always aware of what information we
collect, how we use it, and under what circumstances we disclose
it.
